Please allow 48 hours between dropping your repeat slip in and collecting your prescription.
Requests for repeat prescriptions will not be taken over the phone but you may email to prescriptions.circuitlane@nhs.net

SICKNESS
CERTIFICATE?
The DSS does not require medical certificates for periods
of incapacity lasting seven days or less. For these brief periods of illness a self-certification form
may be obtained from your employer. A medical certificate for illness of less than seven days will
only be issued on a private basis and will incur a fee.
HOW DO I....
MAKE APPOINTMENTS?
For appointments ring 958 2537. Please make it
clear if more than one member of the family needs to be seen. The surgery has an advanced access
appointment system, with a full nurse/doctor triage service. The majority of doctor appointments are
available to book "on the day" though we do reserve a limited amount of appointments for
pre-booking up to two weeks in advance. The doctors would be happy to discuss your problem over the
phone if you feel that you do not need to be seen.
Nurse appointments are pre-bookable up to two weeks in
advance.

OBTAIN TEST RESULTS?
Please phone for test results between 10.00am - 3.45pm,
Monday to Friday.
HOW DO I....
OBTAIN HOME VISITS?
If you cannot possibly get to the surgery, ring 958 2537
before 10.00am to request a home visit. Remember that the doctor can see five or six patients in
the surgery in the time it takes to do one visit. We are therefore very grateful if you can attend.
HOW DO I....
REGISTER?
To register at this surgery, providing you reside within
our boundary, please come in to reception where you will be given a registration form and patient
health questionnaire to complete. You will be asked to make an appointment with the healthcare
assistant for a new patient health check, which takes approximately 10 minutes. You should bring your
completed paperwork to this appointment at which time it will be processed and you will then be
registered.
